If you clicked "show selected items" it will hide them all except the ones you have selected. Press the "S" again to return to normal mode

That would be the TabUI, it's a smaller version of the Right-Shift UI controlled by your arrow keys. You can remove it with .t TabUI

Hard to say but it's 99% guaranteed to be a mod conflict. Possibly visuality-forge-2.0+forge-mc1.19.jar

The main vault hunter mod is a conflict (Vault Hunters X Edition)
