I assume you also have full hunger on the multiplayer server? Do you know what the server type is? (Paper, Bukkit, etc)

paper, other server spigot, on singleplayer it doesnt regenerate me too (i meant that it regenerate me normally not buffed), full hunger

Regen is not guaranteed to work, it works by sending extra packets to the server. If it doesn't work on the server you are playing on then there is nothing we can do. You can try tweaking the speed in the settings for it.
It may also not work reliably anymore on modern Minecraft versions

Yes, its an old mod from 1.8.9 - 1.12.2

if server is 1.8.9 and have viaversion (plugin to play on new versions) should it work?

That would most likely cause even more issues, if it even worked at all. 1.8.9 is extremely unstable and I don’t even know if it is compatible with ViaVersion
